To counteract this, UV photofunctionalization (UV irradiation) will be utilized immediately prior to implant insertion to restore superhydrophilic properties, reduce hydrocarbon content, and enhance early protein adsorption and osteoblastic differentiation.\n\nAdditionally, melatonin (N-acetyl-5-methoxytryptamine) will be investigated as an alternative graft or bioactive material. Melatonin exhibits potent anti-inflammatory and antioxidant capabilities, destroys reactive oxygen species, and stimulates human osteoblasts to synthesize type I collagen fibers. It also down-regulates RANKL-mediated osteoclast activation, thereby reducing bone resorption during the critical early healing phases.\n\nPatients will be selected from the Outpatient Clinic of the Oral and Maxillofacial Surgery Department, Faculty of Dentistry, Mansoura University, following ethical committee approval and obtaining informed written consents. A total of 21 implants will be placed in the posterior maxilla. The clinical and radiographic evaluation will monitor parameters such as primary and secondary implant stability, marginal bone level changes, and peri-implant tissue health to compare the outcomes of photofunctionalized implants alone against those combined with melatonin.",[31,37,42],{"label":32,"type":33,"description":34,"interventionNames":35},"Experimental: UV-Implant + Melatonin","EXPERIMENTAL","Patients in this group will receive ultraviolet (UV) photofunctionalized dental implants combined with the topical application of melatonin hormone at the implant site in the posterior maxilla to evaluate early bone formation.",[36],"Device: UV-Photofunctionalized Dental Implant",{"label":38,"type":39,"description":40,"interventionNames":41},"Active Comparator: UV-Implant Alone","ACTIVE_COMPARATOR","Patients in this group will receive ultraviolet (UV) photofunctionalized dental implants alone without the application of melatonin hormone in the posterior maxilla to serve as the active control group.",[36],{"label":43,"type":39,"description":44,"interventionNames":45},"Conventional Dental Implant","Patients in this group will receive conventional titanium dental implants in the posterior maxilla to serve as the baseline control group.",[46],"Device: Conventional Dental Implant",[48,53],{"type":49,"name":50,"description":51,"armGroupLabels":52,"otherNames":18},"DEVICE","UV-Photofunctionalized Dental Implant","Commercial titanium dental implants subjected to ultraviolet (UV) light irradiation immediately prior to surgical placement to convert the surface property from hydrophobic to superhydrophilic.",[38,32],{"type":49,"name":43,"description":54,"armGroupLabels":55,"otherNames":18},"Standard commercial titanium dental implants placed using traditional surgical protocols without any UV surface photofunctionalization or melatonin application.",[43],[57],{"name":58,"role":59,"phone":60,"phoneExt":18,"email":61},"Ali K Ali, BDS","CONTACT","00201553005571","ak3605773@gmail.com",[63],{"facility":64,"status":65,"city":66,"state":67,"zip":18,"country":68,"countryCode":69,"cosmosGeoPoint":70,"geoPoint":75,"contacts":18},"Faculty of Dentistry, Mansoura University","RECRUITING","Al Mansurah","Dakahlia Governorate","Egypt","EG",{"type":71,"coordinates":72},"Point",[73,74],31.38069,31.03637,{"lat":74,"lon":73},{"type":77,"investigatorFullName":18,"investigatorTitle":18,"investigatorAffiliation":18,"oldNameTitle":18,"oldOrganization":18},"SPONSOR","100658712","clinical-and-radiographic-evaluation-of-uv-treated-dental-implants-with-or-without-melatonin-in-the-upper-posterior-jaw-100658712",false,"NCT07853976","Clinical and Radiographic Evaluation of UV-Treated Dental Implants With or Without Melatonin in the Upper Posterior Jaw","Clinical and Radiographic Assessment of Photofunctionalized Dental Implants With or Without Melatonin in the Posterior Maxilla","UV-MEL-IMPLANT","Inclusion Criteria:\n\n* Patients aged between 18 and 60 years old.\n* Patients seeking replacement of missed single or multiple teeth in the posterior maxilla.\n* Sufficient bone volume (height and width) to accommodate dental implant placement without major bone grafting.\n* Good oral hygiene and motivated patients.\n* Systemically healthy patients with no contraindications for minor oral surgery.\n\nExclusion Criteria:\n\n* Patients with uncontrolled systemic diseases\n* Patients who received radiation therapy in the head and neck region within the last 5 years.\n* Heavy smokers (more than 10 cigarettes per day).\n* Patients with active periodontal disease or poor oral hygiene.\n* Pregnant or lactating females.\n* Parafunctional habits such as severe bruxism or clenching.",true,"ALL","18 Years","60 Years",{"count":91,"type":92},21,"ESTIMATED","INTERVENTIONAL",[95],"NA","The purpose of this study is to evaluate and compare the clinical and radiographic outcomes of using photofunctionalized (UV-treated) dental implants with or without the application of melatonin hormone in the posterior maxilla (upper back jaw).\n\nThe posterior maxilla is known to have lower bone quality, which can negatively affect the primary and secondary stability of dental implants,. UV photofunctionalization is a method that modifies the titanium surface of the implant, changing it from hydrophobic (water-repelling) to superhydrophilic (water-attracting). This enhancement aims to accelerate protein adsorption and promote bone-building cells (osteoblasts). On the other hand, melatonin acts as an anti-inflammatory and antioxidant agent that stimulates bone formation and reduces bone resorption.\n\nA total of 21 dental implants will be placed in patients requiring tooth replacement in the posterior maxilla.
