The study intends to determine the optimal stimulation parameters (frequency, pulse, amplitude) that maximize gait performance while minimizing any potential side effects and to evaluate the effect of CnF DBS on other PD symptoms (including pain) and quality of life.\n\nParticipation in the study is expected to last approximately eight months from enrollment through completion of study activities. Study procedures include medical and neurological evaluations, walking and mobility assessments, questionnaires, brain imaging, device programming visits, and collection of information about your symptoms, safety, and quality of life.\n\nThe most significant risks of participation are those associated with DBS surgery and chronic brain stimulation, including bleeding, infection, stroke, seizure, device-related complications, worsening of neurological symptoms, falls, and stimulation-related side effects.",[29,34],{"label":30,"type":31,"description":16,"interventionNames":32},"Active Stimulation","EXPERIMENTAL",[33],"Device: Active Stimulation",{"label":35,"type":36,"description":16,"interventionNames":37},"Control (Sham or Sub-Therapeutic) Stimulation","SHAM_COMPARATOR",[38],"Device: Sham (No Treatment)",[40,44],{"type":41,"name":30,"description":42,"armGroupLabels":43,"otherNames":16},"DEVICE","Active CnF DBS will be delivered using predefined stimulation parameter ranges considered potentially therapeutic based on prior preclinical and clinical data.",[30],{"type":41,"name":45,"description":46,"armGroupLabels":47,"otherNames":48},"Sham (No Treatment)","Control (sham or sub-therapeutic) stimulation will consist of stimulation settings expected to have no or minimal physiological effect (e.g., stimulation off or amplitude below a predefined threshold).",[35],[49,50],"Control","Sub-Therapeutic",[52,74,91],{"facility":53,"status":16,"city":54,"state":55,"zip":56,"country":57,"countryCode":58,"cosmosGeoPoint":59,"geoPoint":64,"contacts":65},"University of California San Francisco (UCSF)","San Francisco","California","94143","United States","US",{"type":60,"coordinates":61},"Point",[62,63],-122.41942,37.77493,{"lat":63,"lon":62},[66,71],{"name":67,"role":68,"phone":69,"phoneExt":16,"email":70},"Amanda Sayaseng","CONTACT","415-307-2325","amanda.sayaseng@ucsf.edu",{"name":72,"role":73,"phone":16,"phoneExt":16,"email":16},"Doris Wang, MD, PhD","PRINCIPAL_INVESTIGATOR",{"facility":75,"status":16,"city":76,"state":77,"zip":78,"country":57,"countryCode":58,"cosmosGeoPoint":79,"geoPoint":83,"contacts":84},"University of Miami","Miami","Florida","33125",{"type":60,"coordinates":80},[81,82],-80.19366,25.77427,{"lat":82,"lon":81},[85,89],{"name":86,"role":68,"phone":87,"phoneExt":16,"email":88},"Letitia Fisher","305-243-3056","lfisher@med.miami.edu",{"name":90,"role":73,"phone":16,"phoneExt":16,"email":16},"Jonathan Jagid, MD",{"facility":5,"status":16,"city":92,"state":93,"zip":94,"country":57,"countryCode":58,"cosmosGeoPoint":95,"geoPoint":99,"contacts":100},"Philadelphia","Pennsylvania","19107",{"type":60,"coordinates":96},[97,98],-75.16362,39.95238,{"lat":98,"lon":97},[101,105,108],{"name":102,"role":68,"phone":103,"phoneExt":16,"email":104},"James Mitchell","215-829-6720","james.mitchell1@pennmedicine.upenn.edu",{"name":106,"role":68,"phone":103,"phoneExt":16,"email":107},"Neurosurgery Clinical Research Division","npn@uphs.upenn.edu",{"name":109,"role":73,"phone":16,"phoneExt":16,"email":16},"Iahn Cajigas, MD, PhD",{"type":111,"investigatorFullName":112,"investigatorTitle":113,"investigatorAffiliation":5,"oldNameTitle":16,"oldOrganization":16},"SPONSOR_INVESTIGATOR","Iahn Cajigas","Assistant Professor of Neurosurgery and Bioengineering",[115,116,118],{"name":75,"class":6},{"name":117,"class":6},"University of California, San Francisco",{"name":119,"class":6},"Michael J.
