This is a randomized, sequential, blinded, 1:1 paired clinical trial conducted at a single tertiary center. An initial sample of 530 participants was calculated; considering a 10% drop-out, the final calculation was 583 people. All patients will undergo ERCP under sedation or general anesthesia and will be hydrated intravenously with lactated Ringer's. Rectal administration of NSAIDs (indomethacin 100mg) will be performed for all patients without contraindications. In the intervention group, immediately after the end of the therapeutic ERCP, 250 ml of ice-cold distilled water (4º to 5ºC) will be injected towards the papillary region. The control group will undergo standard ERCP and standard pharmacological prophylaxis (indomethacin 100mg), without the instillation of ice-cold distilled water.",[45,51],{"label":46,"type":47,"description":48,"interventionNames":49},"NSAID alone","ACTIVE_COMPARATOR","Standard ERCP, hydration with lactated Ringer's, and standard pharmacological prophylaxis (indomethacin 100mg), without the instillation of ice-cold distilled water.",[50],"Procedure: NSAID alone",{"label":52,"type":53,"description":54,"interventionNames":55},"Cryoprevention associated with NSAID","EXPERIMENTAL","Standard ERCP, hydration with lactated Ringer's, rectal administration of NSAIDs (indomethacin 100mg), and irrigation of the duodenal papilla with 250 ml of ice-cold distilled water (4º to 5ºC).",[56],"Procedure: Cryoprevention associated with NSAID",[58,61],{"type":59,"name":46,"description":48,"armGroupLabels":60,"otherNames":32},"PROCEDURE",[46],{"type":59,"name":52,"description":54,"armGroupLabels":62,"otherNames":32},[52],[64],{"name":65,"affiliation":66,"role":67},"Fauze Maluf Filho","Hospital das Clínicas da Universidade de São Paulo","PRINCIPAL_INVESTIGATOR",[69,74],{"name":70,"role":71,"phone":72,"phoneExt":32,"email":73},"Prof. Dr. Fauze Maluf Filho","CONTACT","+55 11 99191-9014","fauze.maluf@terra.com.br",{"name":75,"role":71,"phone":76,"phoneExt":32,"email":77},"Dr. Mateus Bond Boghossian","+55 11 91795-4571","matbogho@gmail.com",[79],{"facility":80,"status":81,"city":82,"state":82,"zip":83,"country":84,"countryCode":85,"cosmosGeoPoint":86,"geoPoint":91,"contacts":92},"Serviço de Endoscopia Gastrointestinal do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, Brasil.","RECRUITING","São Paulo","05403-000","Brazil","BR",{"type":87,"coordinates":88},"Point",[89,90],-46.63611,-23.5475,{"lat":90,"lon":89},[93,94,95,96,99],{"name":70,"role":71,"phone":72,"phoneExt":32,"email":73},{"name":75,"role":71,"phone":32,"phoneExt":32,"email":77},{"name":65,"role":67,"phone":32,"phoneExt":32,"email":32},{"name":97,"role":98,"phone":32,"phoneExt":32,"email":32},"Mateus Bond Boghossian","SUB_INVESTIGATOR",{"name":100,"role":98,"phone":32,"phoneExt":32,"email":32},"João Guilherme Ribeiro Jordão Sasso",{"type":67,"investigatorFullName":65,"investigatorTitle":102,"investigatorAffiliation":103,"oldNameTitle":32,"oldOrganization":32},"Director of the Endoscopy Unit at the São Paulo Cancer Institute (ICESP) and HCFMUSP","University of Sao Paulo","100649688","cold-and-nsaid-trial-for-prevention-of-post-ercp-pancreatitis-100649688",false,"NCT07738094","Cold and NSAID Trial for Prevention of Post-ERCP Pancreatitis","Comparison Between Local Cryoprevention Associated With NSAIDs Versus NSAIDs Alone for Reducing the Incidence of Post-ERCP Pancreatitis: a Randomized Clinical Trial","Inclusion Criteria:\n\n* Patients aged 18 years or older\n* Clinical indication for ERCP\n* Presence of native major duodenal papilla without previous manipulation\n* Ability of the patient or companion to provide informed consent.\n\nExclusion Criteria:\n\n* Contraindication to ERCP (e.g., severe hemodynamic instability, visceral perforation)\n* Ongoing acute pancreatitis (except mild biliary where ERCP is therapeutic and pain has already resolved)\n* Inaccessibility of the duodenal papilla (e.g., Gastrectomies with Billroth II and Roux-en-Y reconstruction, or gastroduodenal obstructions)\n* Pregnant women\n* Refusal to participate in the study or inability to sign the ICF\n* Contraindication to NSAID use.","ALL","18 Years",{"count":114,"type":115},583,"ESTIMATED","INTERVENTIONAL",[118],"NA","The standard prophylaxis for post-endoscopic retrograde cholangiopancreatography pancreatitis (PEP) is performed with rectal non-steroidal anti-inflammatory drugs (NSAIDs).
