Peripheral nerve blocks are an effective component of multimodal analgesia, and the continuous adductor canal block (CACB) has become a preferred technique because it provides sensory blockade while sparing motor function of the quadriceps, allowing earlier ambulation compared to femoral nerve block. However, CACB does not address posterior knee pain, which can be targeted by the iPACK (infiltration between the popliteal artery and capsule of the knee) block, a technique that blocks terminal sensory branches of the tibial and peroneal nerves without motor blockade.\n\nIn this study, patients scheduled for total knee arthroplasty will be randomized 1:1 (using randomizer.org) to receive either CACB alone (Group 2) or CACB combined with a single-injection iPACK block (Group 1). CACB will be maintained via a perineural catheter connected to a patient-controlled analgesia (PCA) pump delivering 0.1% bupivacaine. Rescue analgesia will be provided with intravenous tramadol (50 mg, max 300 mg\u002Fday) when the numeric rating scale (NRS) pain score exceeds 4.\n\nPain scores (NRS 0-10), total analgesic and opioid consumption, hemodynamic parameters, time to first mobilization (assessed via the Timed Up and Go test), patient satisfaction, and length of hospital stay will be recorded at baseline and at 1, 3, 6, 12, and 24 hours postoperatively by an anesthesiologist blinded to group allocation, different from the anesthesiologist who performed the block.",[62,69],{"label":63,"type":64,"description":65,"interventionNames":66},"Group 1 (CACB + iPACK)","EXPERIMENTAL","Continuous adductor canal block (catheter, PCA with 0.1% bupivacaine) plus a single-injection iPACK block (0.25% bupivacaine 20 mL)",[67,68],"Procedure: IPACK block","Procedure: Continuous adductor canal block (CACB)",{"label":70,"type":71,"description":72,"interventionNames":73},"Group 2 (CACB Alone)","ACTIVE_COMPARATOR","Continuous adductor canal block alone (catheter, PCA with 0.1% bupivacaine)",[68],[75,80],{"type":76,"name":77,"description":78,"armGroupLabels":79,"otherNames":50},"PROCEDURE","IPACK block","Single injection of 0.25% bupivacaine 20 mL between the popliteal artery and posterior knee capsule, performed under ultrasound guidance",[63],{"type":76,"name":81,"description":82,"armGroupLabels":83,"otherNames":50},"Continuous adductor canal block (CACB)","Perineural catheter placement in the adductor canal with PCA-delivered 0.1% bupivacaine (5-7 mL basal rate, 5 mL bolus, 30-min lockout) for 24 hours postoperatively",[63,70],[85,89,92],{"name":86,"affiliation":87,"role":88},"Furkan TONTU, MD","BAŞAKŞEHİR ÇAM VE SAKURA ŞEHİR HASTANESİ","PRINCIPAL_INVESTIGATOR",{"name":90,"affiliation":87,"role":91},"Muzaffer GENCER, MD","STUDY_DIRECTOR",{"name":93,"affiliation":94,"role":91},"Murat Y Mersin, MD","BAŞAKŞEHİR ÇAM VE SAKURA ŞEHİR HASTANESİt",[96,100],{"name":90,"role":97,"phone":98,"phoneExt":50,"email":99},"CONTACT","905059436459","dr.m.gencer07@gmail.com",{"name":86,"role":97,"phone":101,"phoneExt":50,"email":102},"905465771656","furkantontu@gmail.com",[104],{"facility":105,"status":50,"city":106,"state":107,"zip":108,"country":109,"countryCode":50,"cosmosGeoPoint":110,"geoPoint":115,"contacts":116},"Basaksehir Çam Ve Sakura City Hospital, İstanbul, Türkiye","Başakşehir","Istanbul","34480","Turkey (Türkiye)",{"type":111,"coordinates":112},"Point",[113,114],28.791,41.106,{"lat":114,"lon":113},[117,118,122,123,125],{"name":90,"role":97,"phone":98,"phoneExt":50,"email":99},{"name":119,"role":97,"phone":120,"phoneExt":50,"email":121},"Murat Y MERSİN, MD","905367736432","myasinmersin@gmail.com",{"name":86,"role":88,"phone":50,"phoneExt":50,"email":50},{"name":90,"role":124,"phone":50,"phoneExt":50,"email":50},"SUB_INVESTIGATOR",{"name":93,"role":124,"phone":50,"phoneExt":50,"email":50},{"type":88,"investigatorFullName":127,"investigatorTitle":128,"investigatorAffiliation":5,"oldNameTitle":50,"oldOrganization":50},"Furkan Tontu","Doç.
