Diagnosing infection can be challenging, as loosening of the implant cement (osteolysis), damage, and metal reactions can be impossible to differentiate without invasive procedures. Given the significant costs to the NHS for corrective revision surgery, the added suffering and risks to patients from surgery, and the risk of enhancing antimicrobial resistance through the use of broad-spectrum antibiotics, a predictive test for early diagnosis of infection is required. In this study, we will use nanopore sequencing platforms from Oxford Nanopore Technologies (ONT) to identify potential novel biomarkers for PJI in samples collected from patients undergoing revision surgery. This study will explore the microbes present, along with the host's immunological response, to identify potential novel biomarkers in infected samples compared to those revised for other reasons. The investigators will test for detection of these biomarkers in tissue surrounding the implant, as well as in less invasive sample types (synovial fluid and blood). These will provide novel biomarkers for diagnosis and early treatment of PJI for the future.",[32],{"label":33,"type":34,"description":35,"interventionNames":36},"Prosthetic joint Replacement","OTHER","Using nanopore sequencing platforms from Oxford Nanopore Technologies (ONT) to identify potential novel biomarkers for PJI in samples collected from patients undergoing revision surgery",[37],"Procedure: Prosthetic joint replacement",[39],{"type":40,"name":41,"description":42,"armGroupLabels":43,"otherNames":23},"PROCEDURE","Prosthetic joint replacement","Primary joint replacement",[33],[45],{"name":46,"affiliation":47,"role":48},"Samuel Robson","University of Portsmouth","PRINCIPAL_INVESTIGATOR",[50,54],{"name":46,"role":51,"phone":52,"phoneExt":23,"email":53},"CONTACT","+442392848484","samuel.robson@port.ac.uk",{"name":55,"role":51,"phone":56,"phoneExt":57,"email":58},"Erin James","+44 2392 286000","5153","erin.james8@nhs.net",[60,77],{"facility":61,"status":62,"city":63,"state":23,"zip":64,"country":65,"countryCode":66,"cosmosGeoPoint":67,"geoPoint":72,"contacts":73},"Queen Alexandra Hospital","RECRUITING","Portsmouth","PO6 3LY","United Kingdom","UK",{"type":68,"coordinates":69},"Point",[70,71],-1.09125,50.79899,{"lat":71,"lon":70},[74],{"name":55,"role":51,"phone":75,"phoneExt":57,"email":76},"02392 286000","Erin.James8@nhs.net",{"facility":78,"status":79,"city":80,"state":23,"zip":81,"country":65,"countryCode":66,"cosmosGeoPoint":82,"geoPoint":86,"contacts":87},"Southampton General Hospital","NOT_YET_RECRUITING","Southampton","SO16 6YD",{"type":68,"coordinates":83},[84,85],-1.40428,50.90395,{"lat":85,"lon":84},[88],{"name":89,"role":51,"phone":90,"phoneExt":23,"email":91},"Nicholas Norton","+4423 8077 7222","Nicholas.Norton@uhs.nhs.uk",{"type":93,"investigatorFullName":23,"investigatorTitle":23,"investigatorAffiliation":23,"oldNameTitle":23,"oldOrganization":23},"SPONSOR",[95],{"name":47,"class":34},"100657432","detection-of-biomarkers-for-prosthetic-infection-type---debit-100657432",false,"NCT07838064","Detection of Biomarkers for Prosthetic Infection Type - (DEBIT)","DEBIT","Inclusion Criteria:\n\n* Male or Female, aged 18 years or above.\n* Due to undergo prosthetic revision surgery for any reason identified by the clinical team.\n* Participant is willing and able to give informed consent for participation in the study.\n* Pre-operative blood samples and at least 5 substantial peri-prosthetic tissue samples of a suitable size to be distributed between labs as outlined were successfully collected (note that it may not be possible to collect synovial fluid in all cases).\n\nExclusion Criteria:\n\n* The participant has previously undergone any kind of revision surgery for their hip implant.\n* The participant has received any antibiotic treatment within the previous two weeks (excluding antibiotics at induction).\n* Participant lacking capacity to give informed consent.\n* Any other clinical reason the participant should not be included in the study- at the discretion of the clinical team.","ALL","18 Years",{"count":106,"type":107},40,"ESTIMATED","INTERVENTIONAL",[25],"Prosthetic joint infection (PJI) is a significant cause of failure in hip and knee arthroplasty and is often difficult to diagnose using current methods.
