Platelet-rich plasma (PRP) is prepared from autologous blood and contains growth factors that may promote angiogenesis and tissue regeneration. Preclinical studies suggest that PRP may improve erectile function after cavernous nerve injury, but PRP remains an experimental treatment, and no published study has specifically evaluated its effect in men with erectile dysfunction following radical prostatectomy.\n\nThe hypothesis is that intracavernosal PRP injections improve erectile function more than saline injections in men with erectile dysfunction persisting 1-3 years after nerve-sparing radical prostatectomy.\n\nEligible participants will be stratified according to unilateral or bilateral nerve-sparing surgery and randomized 1:1 to receive two intracavernosal injections of either autologous PRP or saline, administered one month apart. An unblinded team member who has no direct interaction with participants will prepare the injections. The participant and the physician administering the injection will remain blinded to treatment allocation. Syringes will be covered with opaque material to maintain blinding.\n\nBlood will be collected before each injection in all participants. For participants assigned to PRP, the blood will be used to prepare the treatment. For the first 10 participants assigned to saline, PRP will also be prepared and analysed to verify the platelet concentration achieved with the preparation procedure. Blood collected from the remaining participants assigned to saline will be discarded.\n\nParticipants must discontinue other treatments for erectile dysfunction for four weeks before baseline and until the assessment one month after the last injection. Thereafter, other treatments for erectile dysfunction will be permitted and recorded.\n\nErectile function and other study outcomes will be assessed at baseline and at 1, 3, 6, and 12 months after the last injection. The primary outcome is the between-group difference in change from baseline in the erectile function domain of the International Index of Erectile Function at one month after the last injection.",[53,59],{"label":54,"type":55,"description":56,"interventionNames":57},"Platelet Rich Plasma (PRP)","EXPERIMENTAL","A volume of 5 mL PRP will be administered in corpus cavernosum on two occasions, with a 1-month interval between injections",[58],"Other: Autologous Platelet Rich Plasma",{"label":60,"type":6,"description":61,"interventionNames":62},"Saline Solution (Placebo)","A volume of 5 mL saline solution will be administered in corpus cavernosum on two occasions, with a 1-month interval between injections.",[63],"Other: saline solution (placebo)",[65,71],{"type":6,"name":66,"description":56,"armGroupLabels":67,"otherNames":68},"Autologous Platelet Rich Plasma",[54],[69,70],"PRP","Platelet-Rich Plasma",{"type":6,"name":72,"description":61,"armGroupLabels":73,"otherNames":74},"saline solution (placebo)",[60],[75],"Placebo",[77],{"name":78,"affiliation":5,"role":79},"Mikkel Fode, MD, PhD","PRINCIPAL_INVESTIGATOR",[81],{"name":82,"role":83,"phone":84,"phoneExt":41,"email":85},"Frederikke Bay Toft, MD","CONTACT","+4538689927","frederikke.bay.toft.01@regionh.dk",[87],{"facility":88,"status":41,"city":89,"state":41,"zip":90,"country":91,"countryCode":92,"cosmosGeoPoint":93,"geoPoint":98,"contacts":99},"Department of Urology, Herlev and Gentofte Hospital","Herlev","2730","Denmark","DK",{"type":94,"coordinates":95},"Point",[96,97],12.43998,55.72366,{"lat":97,"lon":96},[100],{"name":101,"role":83,"phone":102,"phoneExt":41,"email":103},"Mette LK Schmidt, PDH","+4538681100","Mette.Ladefoged.Kopp.Schmidt@regionh.dk",{"type":79,"investigatorFullName":105,"investigatorTitle":106,"investigatorAffiliation":5,"oldNameTitle":41,"oldOrganization":41},"Frederikke Bay Toft","MD","100659388","platelet-rich-plasma-for-erectile-dysfunction-after-radical-prostatectomy-a-randomized-controlled-trial-100659388",false,"NCT07863388","Platelet-Rich Plasma for Erectile Dysfunction After Radical Prostatectomy: a Randomized Controlled Trial","Treatment of Erectile Dysfunction After Radical Prostatectomy With Platelet-Rich Plasma Injections: a Randomized Controlled Trial","Inclusion Criteria:\n\n* Age 40-75 years\n* Underwent unilateral or bilateral nerve-sparing radical prostatectomy between 1 and 3 years prior to the study\n* Good erectile function prior to surgery (IIEF-EF score ≥ 22)\n* Erectile dysfunction at inclusion with an IIEF-EF score \\\u003C 22 without aids\n* Acceptance of a pause in other treatments for erectile dysfunction from 4 weeks before baseline (wash-out) until the 1-month follow-up after the last injection\n* In a stable relationship lasting at least 6 months with a desire for sexual activity\n* Able to understand and accept the study protocol\n\nExclusion Criteria:\n\n* Known psychiatric, neurological and\u002For endocrinological disease requiring medication (including hypogonadism with total testosterone \\\u003C12 nmol\u002Fl).\n* Severe urinary incontinence preventing sexual activity (assessed by a physician at inclusion)\n* Clinical or biochemical recurrence of prostate cancer\n* Other surgery or radiation therapy in the pelvic region\n* Severe cardiac disease in the form of unstable angina, NYHA class \\>II heart failure, uncontrolled arrhythmia, or severe symptomatic and\u002For significant valvular disease\n* Use of anti-androgen medication\n* Peyronie's disease and\u002For previous episodes of priapism\n* Excessive alcohol consumption (more than 21 units per week)","MALE","40 Years","75 Years",{"count":118,"type":119},100,"ESTIMATED","INTERVENTIONAL",[122],"NA","This study will test whether injections of platelet-rich plasma (PRP) can improve erectile function in men with persistent erectile dysfunction (ED) at least 12 months after nerve-sparing radical prostatectomy for prostate cancer.
