Prolonged high-dose corticosteroid therapy can lead to permanent adrenal insufficiency, a fact well-documented in the literature. The human body produces the equivalent of 5-7.5 mg of prednisolone daily; however, data regarding the frequency of adrenal insufficiency associated with the use of therapeutic corticosteroid doses below this endogenous production level are limited. This study aims to investigate the presence of adrenal insufficiency-in accordance with guideline recommendations-among rheumatoid arthritis patients undergoing low-dose corticosteroid therapy (defined as doses not exceeding the equivalent of 5 mg of prednisolone) for short (3 weeks to 3 months), medium (3 to 12 months), and long (more than 12 months) durations. This study also aims to investigate the factors contributing to the development of adrenal insufficiency in rheumatoid arthritis (RA) patients receiving short-, medium-, and long-term corticosteroid therapy.","The study population will be selected from RA patients followed at the Rheumatology Clinic of Bursa Yuksek Ihtisas Training and Research Hospital.",[32,38,42],{"label":33,"type":25,"description":34,"interventionNames":35},"3 week-3 months","low-duration corticosteroid",[36,37],"Diagnostic Test: ACTH stimulation test","Diagnostic Test: Cortisol",{"label":39,"type":25,"description":40,"interventionNames":41},"3-12 months","medium-duration corticosteroid",[36,37],{"label":43,"type":25,"description":44,"interventionNames":45},">12 months","long-duration corticosteroid",[36,37],[47,52],{"type":48,"name":49,"description":50,"armGroupLabels":51,"otherNames":25},"DIAGNOSTIC_TEST","ACTH stimulation test","ACTH stimulation test with 250 micrograms tetracosactide acetate if plasma cortisole is below 10 mcg\u002Fdl",[33,39,43],{"type":48,"name":53,"description":54,"armGroupLabels":55,"otherNames":25},"Cortisol","morning fasting plasma cortisol",[33,39,43],[57],{"name":58,"role":59,"phone":60,"phoneExt":61,"email":62},"Koray Ayar, Associated Proffessor (Dr.)","CONTACT","90 224 2955000","1648","ayarkoray@hotmail.com",[64],{"facility":65,"status":66,"city":67,"state":68,"zip":25,"country":69,"countryCode":25,"cosmosGeoPoint":70,"geoPoint":75,"contacts":76},"SBU Bursa Yüksek İhtisas EAH","RECRUITING","Bursa","Yıldırım","Turkey (Türkiye)",{"type":71,"coordinates":72},"Point",[73,74],29.06013,40.19559,{"lat":74,"lon":73},[77],{"name":78,"role":59,"phone":79,"phoneExt":25,"email":80},"Sabiha Kömoğlu, Dr.","+90 5065990764","sabihakomoglu@gmail.com",{"type":82,"investigatorFullName":83,"investigatorTitle":84,"investigatorAffiliation":85,"oldNameTitle":25,"oldOrganization":25},"PRINCIPAL_INVESTIGATOR","Koray Ayar","Associate Proffessor","Saglik Bilimleri Universitesi","100659496","prevalence-of-adrenal-insufficiency-and-associated-factors-in-rheumatoid-arthritis-patients-using-low-dose-corticosteroids-100659496",false,"NCT07866326","Prevalence of Adrenal Insufficiency and Associated Factors in Rheumatoid Arthritis Patients Using Low-dose Corticosteroids.","Prevalence of Adrenal Insufficiency and Influencing Factors in Patients With Rheumatoid Arthritis and Using Low-dose Corticosteroids.","Inclusion Criteria:\n\n* Participants diagnosed with rheumatoid arthritis according to the 2010 ACR\u002FEULAR criteria.\n* Participants who have been undergoing corticosteroid treatment for at least 3 weeks.\n* Participants in remission or with low disease activity (DAS28-CRP ≤3.2).\n\nExclusion Criteria:\n\n* Patients with primary adrenal insufficiency.\n* Participants who have received pulse steroid therapy within the last 6 months.\n* Pregnancy and lactation.\n* Hypopituitarism\n* Uncontrolled diabetes mellitus\n* Comorbidities; ≥NYHA Class 3 heart failure, Stage 3 chronic kidney failure, ≥Child-Pugh Class B liver cirrhosis","ALL","18 Years","65 Years",{"count":97,"type":98},120,"ESTIMATED","1 Year","OBSERVATIONAL","This study aims to investigate the prevalence of adrenal insufficiency in rheumatoid arthritis (RA) patients receiving low-dose corticosteroid therapy and to identify the factors influencing adrenal insufficiency.
