For those with advanced kidney disease (i.e. approaching the need for dialysis), obesity is especially problematic - obesity can not only lead to worsening kidney function and kidney failure, but it can also prevent patients from receiving a lifesaving kidney transplantation due to concerns about operation risks. Unfortunately, there is an urgent need but no rigorously proven treatments to help people with advanced kidney disease lose weight effectively and safely.\n\nThe investigative team is a large team of doctors, nurses, dieticians, exercise specialists, social scientists, and patients with a lived experience, have spent three years advancing research to help people with advanced kidney disease achieve a healthier body weight. They have created a special weight loss program that combines individualized coaching on diet and save activity with a medication proven to help people with healthier kidneys lose weight (i.e. Ozempic). This research program is called PIVOT: Patient-centred Intervention with Virtual support for Obesity and kidney Transplant access.\n\nThe investigators have started to test PIVOT in a small pilot study and early results show that there is potential to study the program on a much larger scale (i.e. across Canada). With funding from CIHR, the investigators aim to reach more Canadians with advanced kidney disease (including people of lower income, women, and people of different races) with an aim of helping them lose weight safely, feel better, obtain a kidney transplant, and staying healthier over the longer term.",[26,27,28,29],"Obesity","Chronic Kidney Disease","Chronic Renal Disease","CKD","NOT_YET_RECRUITING","2026-09-28",{"date":33,"type":34},"2026-10-05","ACTUAL",{"date":36,"type":20},"2027-01-01",{"date":38,"type":20},"2029-01-01",{"name":40,"class":41},"Kristin Clemens","OTHER",{"id":43,"slug":44,"hasResults":11,"nctId":45,"briefTitle":46,"officialTitle":46,"acronym":47,"eligibilityCriteria":48,"healthyVolunteers":11,"sex":16,"minAge":17,"maxAge":4,"enrollmentInfo":49,"targetDuration":4,"studyType":51,"phases":4,"briefSummary":52,"conditions":53,"keywords":55,"overallStatus":30,"whyStopped":4,"lastUpdateSubmitDate":73,"lastUpdatePostDateStruct":74,"startDateStruct":76,"completionDateStruct":78,"leadSponsor":80,"locationsCount":4},"100604962","research-on-effects-of-drugs-on-body-composition-and-function-with-imaging-in-diabetes-100604962","NCT07156331","Research on Effects of Drugs on Body Composition and Function With Imaging in Diabetes","REBUILD","Inclusion Criteria:\n\n* Be at least 18 years old\n* Have type 2 diabetes\n* Have recently been started on semaglutide or tirzapatide under the care of a physician\n* Be able to speak and read English\n\nExclusion Criteria:\n\n* Are pregnant or breastfeeding\n* Are unable to perform functional tests\n* Are unable to provide informed consent\n* Have any contraindication to PET\u002FCT scan\n* Had a change in body weight over 5 kilograms within 90 days before starting a GLP-1RA or GLP-1RA\u002FGIP\n* Had previous or planned (during the study period) bariatric surgery in the subsequent 12 months.",{"count":50,"type":20},20,"OBSERVATIONAL","One downside of diabetes weight loss drugs like GLP-1RA and GLP-1RA\u002FGIP, is that they can cause muscle loss, which is especially risky for older adults or those already prone to frailty and falls. There is also concern that GLP-1RA might affect bone health, possibly leading to weaker bones and increased risk of fractures. To prevent these issues, doctors need to better predict who is most at risk of muscle and\u002For bone loss. That way, doctors can adjust patient management-like adding targeted physical therapy, or bone-protecting medications alongside GLP-1RA or GLP-1RA\u002FGIP medications.\n\nMedical imaging can help spot early muscle and bone changes. Advanced imaging, like PET and CT scans allow for the assessment of muscle fat content and blood flow in the bones and muscles. This could provide clearer insights into how GLP-1RA or GLP-1RA\u002FGIP medications affect muscles and bones in ways that matter for patients (strength, mobility, falls risk, etc.).\n\nThe investigators will conduct an exploratory study of 20 patients initiating on GLP-1RA or GLP-1RA\u002FGIP by their care provider, and assess their muscle and bone health at baseline, 3, 6, and 12 months using advanced PET\u002FCT imaging. The investigators will also assess functional measures, including grip strength, Timed Up and Go test, and gait speed. The goal is to measure and describe early changes in body composition and physical function that could signal harm. These findings will help to develop a clinical prediction tool for clinicians to use prior to starting semaglutide or tirzepatide, as well as design interventions to help promote healthy muscle and bone while on treatment.",[54],"Type 2 Diabetes",[56,54,57,58,59,60,61,62,63,64,65,66,67,68,69,70,71,72],"T2DM","Semaglutide","Tirzepatide","GLP-1RA","GLP-1RA\u002FGIP","Body Mass","Muscle Composition","Muscle Mass","Mounjaro","Ozempic","Wegovy","DM2","Diabetes Mellitus 2","Muscle Strength","Muscle Function","Muscle Preservation","Bone Health","2025-09-03",{"date":75,"type":34},"2025-09-05",{"date":77,"type":20},"2025-10-01",{"date":79,"type":20},"2027-12-31",{"name":40,"class":41},""]
