and Cutibacterium","Inclusion We will include the group most impacted by PJI\n\n* Adult males\n* Age between 18 and 65 Exclusion\n* History of cryptorchidism, hypospadias, or other congenital abnormality of male reproductive tract\n* Female sex\n* Age younger than 18 or older than 65\n* Current use of Finasteride or other 5-alpha reductase inhibitors\n* Current use of hormone replacement medications such as testosterone\n* Recent antibiotic use within 3 months\n* Have had a negative reaction to oral Finasteride before\n* Recent use of acne treatment within 3 months\n* Self-report very sensitive skin\n* Allergic to chlorhexidine or isopropyl alcohol\n* Have open wounds on both shoulders",true,"MALE","18 Years","65 Years",{"count":20,"type":21},40,"ESTIMATED","INTERVENTIONAL",[24],"PHASE4","High levels of Testosterone causes high amounts of skin bacteria (Cutibacterium Acnes) which can go into the skin during a shoulder replacement surgery and cause a terrible infection.\n\nThe goal of this experiment is to have participants take Finasteride, a medication that blocks testosterone production, for 2 weeks and evaluate the following:\n\n* Blood levels of testosterone\n* Amount of bacteria on the skin.\n\nParticipants will:\n\n* Be assigned to take Finasteride, or a placebo (a look-alike pill with no active medicine) once daily, for 2 weeks\n* Have a small sample of their blood collected and their skin swabbed with a Q-tip to determine Testosterone levels and bacteria on the skin\n* Blood draw and skin Patients","Inclusion Criteria:\n\n* Adult male 18 years of age or older undergoing elective primary total shoulder arthroplasty, including both anatomic and reverse total shoulder arthroplasty, for indications including massive rotator cuffs, rotator cuff arthropathy, glenohumeral osteoarthritis, and acute fractures.\n\nExclusion Criteria:\n\n* Any revision shoulder arthroplasty procedure or other shoulder surgery that is not a primary shoulder arthroplasty.\n* Patients with allergy to active ingredients in Prontosan gel\n* Patient is unable to consent for themselves\n* Patient is Disease and the Clinical Effect of Microdiscectomy","Inclusion Criteria:\n\n* minimum age of 18 years\n* MRI of the lumbosacral spine performed one month ± 2weeks before surgery showing a disc protrusion, herniation, or free nucleus pulposus sequestration\n* matching physical examination findings including positive straight leg raise test, dermatomal sensory deficits, myotome motor deficits, and diminished deep tendon reflexes.\n\nExclusion Criteria:\n\n* antibiotics or corticosteroid use the month before surgery\n* ongoing bacterial or viral infection\n* immunocompromised
