Gastric emptying will be objectively measured using bedside point-of-care ultrasound - a safe, painless imaging method - at three time points: before feeding (T0), at 1 hour (T1), and at 2 hours (T2) after restarting enteral nutrition following a 2-hour fasting period.

Patients will be stratified into three groups based on norepinephrine equivalent dose (NEQ): Group 1 (no vasopressor, NEQ=0), Group 2 (low-to-moderate dose, NEQ \\>0 to <0.3 μg/kg/min), and Group 3 (high dose, NEQ ≥0.3 μg/kg/min). The primary outcome is gastric emptying rate (GER = (T0 volume - T2 volume) / 2 hours). Gastroparesis is defined as GER ≤0 mL/hour. Secondary outcomes include antrum cross-sectional area, agreement between ultrasound and aspirated gastric residual volume (Bland-Altman analysis), and enteral nutrition intolerance incidence. A subgroup analysis by shock type (septic, cardiogenic, hypovolemic, obstructive) will also be performed.

The study aims to identify the vasopressor dose threshold at which gastric emptying is significantly impaired, and to support the use of bedside gastric ultrasound as an objective, non-invasive monitoring tool for enteral nutrition management in the ICU.",[25,26,27,28,29],"Gastroparesis","Shock","Critical Illness","Gastric Emptying","Enteral Nutrition Intolerance (EFI)",[31,32,33,34,35,36,37,38,39,25],"Gastric ultrasound","Point-of-care ultrasonography","Gastric emptying rate","Vasopressor","Norepinephrine","Antrum cross-sectional area","Gastric residual volume","Critical illness","Intensive care unit","RECRUITING","2026-09-01
2026-09-03","ACTUAL
2026-09
2027-11
Başakşehir Çam & Sakura City Hospital","OTHER_GOV",1
"100632777","serum-bicarbonate-and-enteral-feeding-intolerance-in-adult-icu-a-prospective-clinical-trial-100632777","NCT07518095","Serum Bicarbonate and Enteral Feeding Intolerance in Adult ICU: A Prospective Clinical Trial","Serum Bicarbonate and Enteral Feeding Intolerance in Adult ICU Patients: A Prospective Clinical Trial","Inclusion Criteria:

* Adult patients aged 18 and older.
* Patients started on enteral nutrition (EN) within the first 48 hours of their intensive care unit admission.
* Patients predicted to remain in the intensive care unit for at least 72 hours.
* Patients (or their legal representatives) who have provided consent to participate in the study.

Exclusion Criteria:

* Patients under the age of 18.Individuals who are currently pregnant or breastfeeding.
* Patients with gastrointestinal (GI) perforation, active GI bleeding, or mechanical bowel obstruction.
* Patients who have undergone major abdominal surgery (e.g., esophagectomy, gastrectomy) within the last month.
* Patients receiving total parenteral nutrition (TPN) or those who are only fed orally.
* Patients requiring specialized enteral formulas (e.g., diabetic, renal, immunomodulatory, elemental, or semi-elemental) due to clinical necessity.
* Patients with a known diagnosis of diabetes or those with HbA1c \\>6.5 requiring a diabetic enteral formula.
* Patients with terminal-stage malignancy or those requiring specific enteral nutrition protocols.
* Patients with advanced (Child-Pugh C) liver failure.
* Patients with chronic kidney failure requiring dialysis.
* Patients with diabetic ketoacidosis, severe metabolic asidozis, or those requiring initial bicarbonate replacement.
* Patients who are brain dead or in the palliative care stage.
70,"This prospective, observational study evaluates whether serum bicarbonate HCO3 levels can predict enteral nutrition intolerance (EFI) in adult ICU patients. Objective: To determine if lower HCO3 levels correlate with the development of EFI.Design: A 12-month study involving 70 participants at Sultangazi Haseki Training and Research Hospital.Methodology: Patients starting enteral nutrition within 48 hours of admission will be monitored for 72 hours.
