Men and women with an age of 18 years or older (according to local labeling).\n2. Willing to participate in the study (i.e., signed ICF). (Willing to be randomized).\n3. Patient is able to undergo a high-resolution Computerized Tomography (CT) scan.\n4. Patient is able to fit into MRI unit and comply with all contraindications for the specific magnetic resonance (MR) system including and limited to contrast medium should there be needed.\n5. The thalamus, sub-thalamus and the pallidum must be apparent on MR imaging.\n6. Twelve adults with clinically diagnosed ET will undergo six LIFUP treatment sessions over approximately two weeks using the BrainSonix BX Pulsar 1002 system. Tremor severity will be assessed using clinician-rated scales (TETRAS and FTM), patient-reported quality-of-life measures (QUEST), and objective accelerometry before and after treatment sessions, with additional follow-up visits at one and three months post-treatment. MRI scans will be performed at baseline and after the final treatment session to monitor safety. This will allow researchers to examine how stimulation affects both the learning of new speech sequences and the production of previously learned sequences. Proof of this is not required for patients with a current device that is being removed and replaced with a new device. An adequate medication trial is defined as a therapeutic dose of each medication or the development of side effects as the medication dose is titrated.\n* Patient is available for appropriate follow-up times for the length of the study.\n\nExclusion Criteria:\n\n* Any previous neurosurgical intervention other than VIM DBS, including ablative brain lesions for tremor suppression.\n* Medication related movement disorders.\n* Any suspicion of Parkinson's disease, including presence of Parkinsonian features such as bradykinesia, rigidity, or postural instability.\n* Any behaviors consistent with alcohol or substance abuse as defined by the criteria outlined in Diagnostic and Statistical Manual of Mental Disorders (DSM-V).\n* Severe medical co-morbidity including cardiovascular disorder, lung disorder, kidney disease, continuous neurological disease, hematological disease, or frailty that impact tolerability of the surgery as judged by the screening physicians.\n* Abnormal brain MRI including hydrocephalus, stroke, structural lesions, demyelinating lesions, or infectious lesions.
