Based on a standard 12-lead electrocardiogram (ECG), patients can be diagnosed with ST-segment elevation myocardial infarction (STEMI), which requires immediate medical treatment. Recently, wearable devices like smartwatches with ECG capabilities have opened new pathways for cardiac triage, but their diagnostic precision and technical viability in a real, unselected Emergency Department setting need to be confirmed.\n\nThe primary objective of this prospective, observational study is to compare the diagnostic capacity of a 9-lead ECG obtained with a smartwatch against the standard 12-lead ECG for diagnosing STEMI in patients consulting for chest pain.\n\nDuring the study, patients evaluated by the on-call cardiologist for ischemic chest pain will undergo their standard care. In addition to the standard 12-lead ECG, a 9-lead ECG will be sequentially recorded using a smartwatch. This is an observational study, meaning the smartwatch recording will not cause any delay in clinical action or negatively impact the patient's standard treatment.\n\nSpecifically, the study aims to:\n\n* Analyze the diagnostic agreement between expert cardiologists when blindly and independently classifying the smartwatch recordings versus the standard ECG recordings as \"STEMI\" or \"non-STEMI\".\n* Evaluate the technical feasibility of using the smartwatch in an Emergency Department setting by assessing the percentage of unreadable records due to artifacts.",[25,26],"STEMI - ST-segment Elevation Myocardial Infarction","Chest Pain",[28,26,29,30,31],"ST Elevation Myocardial Infarction","Smartwatch","Wearables","Electrocardiography","RECRUITING","2026-08-24",{"date":35,"type":36},"2026-08-26","ACTUAL",{"date":38,"type":36},"2026-08-14",{"date":40,"type":21},"2027-02",{"name":42,"class":43},"Hospital General Universitario Santa Lucia","OTHER",1,{"id":46,"slug":47,"hasResults":11,"nctId":48,"briefTitle":49,"officialTitle":50,"acronym":51,"eligibilityCriteria":52,"healthyVolunteers":11,"sex":17,"minAge":18,"maxAge":53,"enrollmentInfo":54,"targetDuration":4,"studyType":56,"phases":57,"briefSummary":59,"conditions":60,"keywords":61,"overallStatus":65,"whyStopped":4,"lastUpdateSubmitDate":66,"lastUpdatePostDateStruct":67,"startDateStruct":69,"completionDateStruct":71,"leadSponsor":73,"locationsCount":4},"100576602","phase-4-effects-of-nicorandil-on-microvascular-dysfunction-in-patients-with-stemi-undergoing-primary-pci-100576602","NCT06787430","Effects of Nicorandil on Microvascular Dysfunction in Patients With STEMI Undergoing Primary PCI","Evaluating the Effects of Nicorandil on Microvascular Dysfunction in Patients With ST-segment Elevation Myocardial Infarction Undergoing Primary Percutaneous Coronary Intervention","NORMAL","Inclusion Criteria:\n\n* Age between 18-80\n* Diagnosis of STEMI and indicating for primary PCI\n* Diagnostic coronary angiography showed definite culprit vessel and primary PCI was planned to treat the culprit vessel.\n* Reference vessel diameter of culprit vessel was greater than 2.0 mm\n* There was no flow-limiting stenosis for the non-culprit vessels.\n* Understand the aim of this trial and agree to sign the informed consent form.\n\nExclusion Criteria:\n\n* Acute in- stent thrombosis or in-stent stenosis lesions of culprit vessel.\n* Balloon angioplasty rather than stent was planned for the culprit vessel.\n* Allergic to nicorandil or systolic arterial pressure was less than 85mmHg before procedure\n* Vessel with twisted shape that was unable to be measured using AMR\n* Oral or intravenous use of nicorandil within one month.\n* Cardiac shock needing mechanical support.\n* Severely hepatic dysfunction.\n* Severely renal failure needing hemodialysis.\n* Contraindicated for coronary angiography or primary PCI.\n* Culprit vessels of left main or graft vessels.\n* Pregnant or nursing.\n* Others that investigators think should excluded.","80 Years",{"count":55,"type":21},170,"INTERVENTIONAL",[58],"PHASE4","Despite the success of restoration of epicardial blood flow by the primary percutaneous coronary intervention (PPCI), approximately one half of patients with ST-segment elevation myocardial infarction (STEMI) have failed myocardial reperfusion, namely microvascular dysfunction. Previous studies have shown that coronary microvascular damage significantly impacts the prognosis of patients with STEMI. Therefore, restoration of microvascular function is quite important during the PPCI procedures. Recent clinical trials found that nicorandil, a hybrid ATP-sensitive potassium channel opening agent, improved microvascular function, prevented no-reflow phenomenon, and had beneficial cardioprotective effects in patients receiving PPCI. However, there was a lack of evidence on the protective effects of nicorandil on microvascular function. The coronary angiography-derived IMR (AMR) is a novel wire-free technology developed for fast computation of IMR in patients undergoing coronary angiography, thereby enabling the quantification of coronary microcirculation of patients. This study is aimed to investigate the effects of nicorandil on microvascular dysfunction as evaluated by AMR in patients with STEMI.",[25],[62,63,64],"Nicorandil","STEMI","microvascular dysfunction","NOT_YET_RECRUITING","2026-03-11",{"date":68,"type":36},"2026-03-12",{"date":70,"type":21},"2026-03-01",{"date":72,"type":21},"2027-12-31",{"name":74,"class":75},"China National Center for Cardiovascular Diseases","OTHER_GOV"]
