Development and Validation of the Indian Asthma Care Bundle for Reducing Readmission and Improving Post-hospitalisation Care After Discharge From Hospital Due to an Acute Asthma Exacerbation

The goal of this study is to assess the effectiveness of the Indian Asthma Care Bundle (I-ACB) in improving post-discharge asthma management among patients with asthma in India, specifically targeting adults and children experiencing severe asthma exacerbations. The main questions it aims to answer are: 1. Does the I-ACB reduce the rate of hospital readmissions within 30, 60, and 90 days post-discharge? 2. How does the I-ACB impact treatment adherence, lung function, symptom control, and overall quality of life, compared to usual care? Researchers will compare outcomes among participants receiving the I-ACB with those receiving standard asthma care (usual care) to determine whether the I-ACB improves health outcomes and reduces the need for emergency department visits. Participants will be asked to: * Engage in a structured asthma management program that includes education on inhaler techniques and adherence to treatment plans. * Report their symptoms and treatment adherence regularly. * Participate in follow-up assessments that monitor lung function and quality of life over a specified period. The importance of this study lies in its potential to address significant gaps in asthma management in India, which currently lead to high hospitalisation rates and substantial healthcare costs. By developing a tailored care strategy, the I-ACB aims to improve health outcomes for millions of asthma patients, reducing the burden on families and healthcare systems alike, and ultimately transforming the approach to asthma care in the region.

Efficacy of High-Dose Continuous Intravenous Magnesium Sulfate Infusion vs Standard Intravenous Magnesium Sulfate Therapy in Status Asthmaticus in the ED

The goal of this clinical is to compare treatment outcomes between high dose continuous IV Magnesium and standard therapy in the management of acute exacerbation of asthma in the emergency department. Participants will random assign to either high dose continuous IV drip or standard infusion. Participants in the intervention group received high-dose 50% magnesium sulfate (MgSO4), consisting of 2 g + normal saline (NSS) 100 ml administered intravenously over 30 minutes, followed by MgSO4 6 g + NSS 350 ml infused over 3.5 hours (approximately 40-50 mg/kg/h). In contrast, participants in the control group received standard-dose 50% MgSO4, consisting of 2 g + NSS 100 ml administered intravenously over 30 minutes, followed by a NSS 350 ml infusion over 3.5 hours. Measurements include symptom severity, respiratory rate, oxygen saturation, and pulmonary function tests. Primary endpoints are ED stay length, hospital admission, and ED revisit within 48 hours post-discharge. Adverse events are documented.

Evaluation of the Medentum Diagnostik Platform for Pediatric ENT Conditions and Asthma

This study evaluates the diagnostic performance, usability, and clinical utility of the Medentum Diagnostik platform, a multisensor device with integrated software and artificial intelligence, in pediatric patients with asthma and ENT conditions. Participants will be assigned to either home-based use of the Diagnostik platform with telehealth-supported clinician evaluation or standard in-person care. The primary objective is to assess agreement between AI-generated diagnoses and clinician diagnoses obtained during study encounters, including telehealth evaluations in the Diagnostik arm and in-person evaluations in the standard-of-care arm. Secondary objectives include evaluating agreement between AI-generated diagnoses and independent research team diagnoses, as well as assessing usability, patient and caregiver satisfaction, healthcare utilization, and clinical outcomes such as asthma control and symptom resolution. This study will also evaluate differences in healthcare utilization, including in-person visits, emergency care use, and time to diagnosis and treatment, between study groups.

Pollution Intervention to Impact Kids Asthma Study

The goal of this clinical trial is to learn if adding Air Quality Index (AQI) information to asthma action plans works to improve asthma outcomes in children. It will also learn about children with asthma who report being more sensitive to outdoor air pollution. The main questions it aims to answer are: * Does adding either information about the EPA-AQI or commercial AQI improve asthma outcomes over time? * Are there changes in nasal gene expression in children with asthma who report they are more sensitive to outdoor air pollution? Researchers will compare EPA-AQI and the commercial-AQI groups to a control group to either AQI works to improve asthma. Participants will: * Receive standardized outdoor air pollution education and an asthma action plan * Provide nose and blood specimens * Have visits every 4 weeks for 48 weeks, 10 will be conducted by telephone calls and 3 visits will be in person.
