Horizontal Ridge Deficiency

Single implant sites with bone loss can often be managed by bone grafting alone. The purpose of this study is to determine whether putting a soft tissue graft substitute over the bone graft will provide an increased benefit for improving the health, appearance, and stability of the bone and gums surrounding the implant.

To assess the accuracy of the computer assessed guides in horizontal bone augmentation achieved at posterior mandible using guided modified ridge splitting technique versus free hand modified ridge splitting technique

Khoury technique, which employs autogenous bone blocks harvested from the patient, has been widely used for horizontal and vertical ridge augmentation. This method introduces critical limitations, including the technical complexity, morbidity associated with donor site harvesting, such as infection, neurosensory disturbances, and post-operative discomfort This clinical case series aims to demonstrate how customized pours zirconium shell could be an alternative technique for ridge augmentation to overcome the limitations of Khoury shell technique.

The protocol outlines a randomized clinical trial comparing two surgical techniques for horizontal ridge augmentation in the anterior mandible: Computer-Guided Autogenous Cortical Shell Technique (using a patient-specific guide for precise placement). Free-Hand Autogenous Cortical Shell Technique (traditional approach without a guiding template). Key Points: The study aims to determine which method provides better accuracy and bone volume stability. Cone Beam CT scans (CBCT) will assess horizontal bone gain preoperatively and six months postoperatively. The trial is conducted at Cairo University's Faculty of Dentistry. Participants: Patients with pure horizontal bone loss in the anterior mandible. Outcomes: Primary: Bone width gain in millimeters. Secondary: Accuracy of augmentation and surgical time. Methodology: The study group will use computer-designed surgical osteotomy guides to harvest and place bone grafts with improved precision. The control group will rely on the conventional free-hand approach. Both groups will undergo bone grafting using autogenous cortical bone shells. Data Collection \& Analysis: Participants will be randomized and blinded. Data will be analyzed using statistical methods to compare effectiveness.

To evaluate the quantity and quality of bone generated, radiographically and histomorphometrically, after guided bone regeneration (GBR) using a membrane of 3D printed custom made (PMMA) and subsequent stability of delayed implant placement.

The current study aimed to evaluate both the quality and quantity of gained and maintained bone width after ridge splitting utilizing autogenic bone wedges versus mixed bone particles for horizontal ridge augmentation in the anterior esthetic zone.
