Clinical Characteristics and In-Hospital Outcomes of Critically Ill Patients Receiving Hemodialysis or Continuous Venovenous Hemodiafiltration in the Emergency Department

This retrospective observational study aims to evaluate the clinical characteristics and in-hospital outcomes of critically ill adult patients who underwent hemodialysis (HD) or continuous venovenous hemodiafiltration (CVVHDF) in the Emergency Department of Antalya Training and Research Hospital between January 1, 2023, and August 1, 2026. Medical records will be reviewed to collect information on patient demographics, comorbidities, clinical findings at admission, vital signs, renal replacement therapy indications, laboratory results before and after treatment, treatment modality, treatment-related complications, need for intubation or vasopressor/inotropic support, intensive care unit admission, length of stay, and hospital outcomes. The primary outcome is in-hospital mortality. Secondary outcomes include emergency department mortality, intensive care unit admission, ward admission, discharge status, emergency department and hospital length of stay, need for intubation or vasopressor/inotropic support, treatment completion, treatment-related complications, and changes in laboratory parameters before and after renal replacement therapy. No additional intervention, test, treatment, or biological sample collection will be performed as part of this study.

Development and Validation of an Ultrasound-Based Difficult Cannulation Risk Score for Pre-procedural Internal Jugular Vein Catheterization in Critically Ill Patients: A Prospective Observational Cohort Study

Ultrasound-guided internal jugular vein (IJV) catheterization is widely used in critically ill patients. Despite ultrasound guidance, difficult catheterization and first-pass failure remain clinically important and may increase the risk of mechanical complications. Pre-procedural characteristics of the IJV, its anatomical relationship with the common carotid artery (CCA), head rotation, and operator experience may influence procedural success. The ROTATE-ICU study is a prospective observational cohort study designed to investigate pre-procedural ultrasound and clinical predictors of difficult IJV catheterization in critically ill adults. Consecutive adult intensive care unit patients undergoing ultrasound-guided IJV central venous catheterization will undergo standardized pre-procedural ultrasound assessment. IJV anterior-posterior diameter, skin-to-vessel depth, IJV position relative to the CCA, head rotation angle, operator experience, and relevant clinical and procedural characteristics will be recorded. The primary objective is to develop and internally validate a clinical prediction model for cannulation difficulty and first-pass cannulation outcome. Secondary objectives include evaluating the relationship between head rotation and IJV morphology and describing the association between complete IJV collapse and procedural outcomes. The planned enrollment is 150 participants.

Predictive Performance of NEWS-2 and NEWS-Calcium Scores for In-Hospital Mortality in Older Patients With Acute Pancreatitis

Acute pancreatitis is an important gastrointestinal emergency that may lead to mortality, particularly in older patients. This retrospective, single-center observational prognostic validation study aims to compare the predictive performance of the National Early Warning Score 2 (NEWS-2) and NEWS-Calcium scores for in-hospital mortality in patients aged 65 years and older diagnosed with acute pancreatitis in the emergency department. Electronic medical records of eligible patients will be reviewed retrospectively. Demographic characteristics, vital signs, serum calcium levels, clinical outcomes, intensive care unit admission, organ failure, length of hospital stay, and in-hospital mortality will be recorded. NEWS-2 will be calculated using vital parameters at admission, while the NEWS-Calcium score will be obtained by adding a calcium-based score to NEWS-2. The primary outcome will be in-hospital mortality. Secondary outcomes will include intensive care unit admission, severe acute pancreatitis, persistent organ failure, and length of hospital stay. The predictive performances of the scores will be compared using ROC curve analysis, AUC values, and the DeLong test. This study aims to identify the most appropriate early prognostic score for older patients with acute pancreatitis and to support clinical decision-making in emergency settings.
