A Pilot Evaluation of Refractive Error Progression in Pre-Myopic Children in Singapore With and Without Myopia Control Spectacles

This study aims to find out whether ZEISS MyoActive lenses can help prevent short-sightedness from developing in children. We aim to recruit at least 50 children in total for this study. Myopia (short-sightedness) is becoming increasingly common in children worldwide. Early onset of myopia often leads to faster progression and a higher chance of developing high myopia, which increases the risk of serious eye conditions that can affect vision in adulthood, such as retinal detachment, glaucoma, and cataracts. Current treatments can slow the worsening of short-sightedness once children become myopic, but there is limited evidence on whether early intervention can delay myopia from developing in the first place. This study will compare two types of commonly used spectacle lenses: ZEISS MyoActive lenses, which are designed to provide a type of optical signal that may reduce the worsening of short-sightedness; and Standard single-vision lenses, which are the usual lenses prescribed for clear vision. Children who join the study will be randomly assigned to wear one of these two lens types for 12 months. They will attend regular check-ups during this time so we can monitor their eye health and vision. By randomly assigning children to one of these two lens types and following them over 12 months, we aim to determine whether ZEISS MyoActive lenses: reduce the likelihood of developing short-sightedness, slow changes in how short-sighted a child's eyes are becoming and eye growth, and are safe, comfortable, and well-tolerated for everyday use. The findings from this study may help improve early myopia prevention strategies and potentially reduce future risks of high myopia and its associated eye conditions.

Predictors of Vitrectomy Outcomes in Myopic Foveoschisis

This prospective pilot study evaluates factors that predict outcomes after vitrectomy for myopic foveoschisis (MFS), a sight-threatening complication of high myopia that can lead to progressive visual loss, foveal detachment, macular holes, and retinal detachment. Although vitrectomy can relieve traction and improve retinal anatomy, the optimal timing of surgery and the factors associated with visual recovery remain uncertain, particularly in patients with relatively good pre-operative vision. The study will recruit 30 adults aged 40-90 years from Singapore National Eye Centre surgical retina clinics. Participants must have high myopia (worse than -6 dioptres and axial length \>26 mm) with MFS requiring surgery because of progressive vision loss with OCT-confirmed deterioration over six months and/or foveal detachment. Patients with other ocular diseases or conditions that could confound imaging or functional testing, poor surgical candidacy, cognitive impairment, or ungradable imaging will be excluded. Participants will undergo standard-of-care 23- or 25-gauge pars plana vitrectomy, with fovea-sparing internal limiting membrane peeling or flap, possible gas tamponade, and cataract surgery where clinically indicated. The research does not alter the usual surgical approach; its main purpose is to collect structural and functional outcome data over 12 months. Assessments will be performed at baseline and during postoperative follow-up, including best-corrected visual acuity, refracted logMAR visual acuity, microperimetry, M-chart testing for metamorphopsia, contrast sensitivity using the Manifold Contrast Vision Meter, and swept-source OCT. OCT measures will include central retinal thickness, foveal detachment, inner and outer macular schisis, lamellar holes, and epiretinal membrane. Demographic and clinical variables, including age, sex, co-morbidities, and lens status, will also be collected. The primary functional outcome is stable or improved best-corrected visual acuity. Anatomical success is defined as improvement in retinoschisis and resolution of pre-existing foveal detachment. The investigators hypothesise that improvement can occur across a range of MFS severities, and that baseline OCT features and functional measures can help predict 12-month surgical success. As a pilot study, no formal sample-size calculation is planned. Analyses will describe clinical characteristics, compare outcomes between better and poorer pre-operative visual-acuity groups, and use multivariable logistic regression to identify prognostic factors. Risks are limited to usual surgical complications and confidentiality risks; all study tests are non-invasive. Data will be securely stored, access restricted to the study team, and participants will provide informed consent. Parallel studies will be conducted with overseas collaborators.

Faricimab for Chronic Central Serous Chorioretinopathy: A Randomized Sham-Controlled Trial

Dysregulation of the Angiopoietin-2 (Ang-2)/Tyrosine kinase with Immunoglobulin-like and EGF-like domains 2 (Tie-2) signaling pathway has been implicated in choroidal vascular instability and RPE dysfunction in Central serous chorioretinopathy (CSCR) and pachychoroid-associated neovascularization. This study prospectively evaluates the efficacy and safety of faricimab compared to sham in CSCR with and without secondary neovascularization, using standardized anatomical and functional endpoints. The results of this trial may help define the role of dual pathway inhibition in CSCR and inform future treatment strategies for this challenging and vision threatening condition.

Predicting Risk of Progression of Early to Late AMD in the Aging Eye Through Imaging and Multimodal Evaluation

Progression Patterns of Choroidal Venous Alterations

This study is about understanding how a condition called pachychoroid changes over time. Pachychoroid refers to an anatomic choroidal characteristic with abnormally thick choroid or abnormally dilated Haller's vessels. Pachychoroid can range from mild forms which does not cause any problems to more serious ones that might cause vision loss.This study will recruit participants with pachychoroid, to learn more about how this condition develops, how it affects people in the long run, and what factors might make it worse.

Detection and Optimization of Treatment of Severe Cases of Dry Eye Disease

The bulk of dry eye patients are found in the community. The lack of satisfactory protocols and confidence is a significant deterrent for practitioners to manage such patients, which may result in inaccurate referrals, and unhappy patients. Problems are compounded by comorbidities of dry eye, even if these are not diagnosed formally. Aligning with the healthcare strategy to move beyond healthcare to health, and beyond hospital care to community care, investigators propose that the confidence of primary carers be increased by using an image-based screening system. This study aim to determine the efficacy of this screening AI algorithm, a prototype, in addition to or instead of screening of dry eye using a simple DEQ-5 symptom questionnaire.

Telemedicine for Evaluating Dry Eye Disease (DED) Using a Mobile Phone-Attached Portable Automatic Ocular Surface Imaging Device (PAOSID): A Patient-Operated Diagnostic and Continuous Ocular Surface Monitoring (COSM) System

Dry eye disease (DED) is a common eye condition that is becoming more widespread. Detecting it early, keeping track of its progression, and following up regularly can help protect vision and prevent serious complications. However, due to a shortage of ophthalmologists, limited access to eye care services, and disparities in care quality, many patients receive infrequent or insufficient clinical consultations. To address these needs, a portable automatic ocular surface imaging device (PAOSID) that attaches to a mobile phone has been developed. This device uses three types of light including white, infrared, and cobalt blue light to capture clear images and videos of the eye's surface. It also has a smart system that automatically takes high-quality images, allowing patients to use it at home independently. This device may help detect early signs of DED and monitor eye health more easily.

Comparing Tear Proteomics Profile in Dry Eye Disease pre-and Post-treatment With Low Level Light Therapy

The purpose of this research study is to understand the mechanism of action of low level light therapy and also potentially yield markers associated with good treatment response. Low-level light therapy is light-based treatment delivered through advance eye-light device. This therapy uses light energy to provide energy to the meibomian gland cells to slow down age-ing, improve tear stability and reduce inflammation.

Identification of Molecular Signals in Vitreous Humor Associated With Suboptimal Response to Vascular Endothelial Growth Factor (VEGF) Inhibition in Neovascular Age-related Macular Degeneration (nAMD) Within a Clinical Trial Setting

Neovascular age-related macular degeneration (nAMD), also called wet AMD, can cause serious vision loss. While anti-VEGF (anti Vascular Endothelial Growth Factor) treatments such as ranibizumab help many patients, about 20 40% have a suboptimal response. In this study, the investigators want to identify other factors (beyond VEGF) that might be driving the disease in these non-responding patients. By looking at samples from inside the eye (vitreous humor) and comparing "good responders" to "suboptimal responders", the investigators hope to find potential new treatment approaches or biomarkers for nAMD.
